REQUIREMENTS FOR NEW PASSPORT:

Complete and sign passport application form

Attach three recent passport size photos identically signed by a "justice of peace". (Attach the photos to the form using cellophane tape).

 

New Passport Processing Fees:

All fees must be paid by cash.

 

REQUIREMENTS FOR PASSPORT EXTENSION:

Write a letter requesting an extension of your passport informing of the requested years of duration and sign the letter.    

Attach three recent passport size photos.

Your original passport.

 

Passport renewal processing fees:  

Passports can be renewed for a maximum of five years. They are renewed from the date of expiry.

 

Extension fees are:

1 year

2 years

3 years

4 years

5 years

 

All fees must be paid by cash.

 

If you are applying by mail, please include a prepaid, self-addressed return envelope, large enough for the return of your passport and documents. You may use Certified or Registered mail, or any other traceable mailing service or courier to ensure safety of your passport).

 

IMPORTANT INFORMATION:

New passports issued are valid for five years only. You can not apply for any shorter or longer duration of validity. Individuals must carry their own passports. Therefore, the spouse and the children cannot be included in the same passport as their parents.
